Community Support Request Forms
These two forms help us serve people with clarity, care, and integrity. The Personal Assistance Form is for individuals or households seeking support with practical needs such as employment help, housing resources, clothing, hygiene items, or other essentials. The Event Support Form is for organizers requesting assistance or resources for a community event. Each form allows us to understand real needs, steward resources wisely, and follow up appropriately. All information submitted is kept confidential and reviewed with prayerful consideration so we can respond effectively and responsibly.
At CROSSWAY CHURCH, we have a vision to not only birth, sustain and spread REVIVAL to those who visit our church, but to also bring that fire and zeal to the streets of Phoenix, Arizona. We have a vision to one day have a facility of our own that would be able to house those in need. Let's partner in together to help those who have no food or clothes and just need a loving hand up and not a hand out.
